"What if" NK actually fires a Nuclear weapon at Guam?

North Korea has a giant battery of heavy artillery pointed at Seoul. They could kill thousands, and level much of the city, within hours, without even bothering with nuclear weapons.
I don't think the US would automatically unload nukes on NK unless NK started shelling population centers with that artillery. At that point, it would be a race to destroy as much of the artillery as possible to reduce casualties in South Korea. Given the hilly terrain, and the fact that NK has spent the last half century encasing their artillery and missile launchers within hills, pill boxes, fortified bunkers, etc., nukes might be justified.
